Transferring Members

To transfer to Geelong Ostomy download the form using the link button above, Fill out the form and email to: contactus@geelongostomy.com.au

We will contact you by email or phone to indicate that your transfer has been successful.

If any further information or payments are required we will contact you before you can place your first order with Geelong Ostomy.

If you have paid your Stoma Access Fee/Membership at your former Association, you will not be required to pay addition fees for the current financial year.

Postal orders must be pre-paid by making a direct debit into our bank account, or credit card payment. Contact Geelong Ostomy via contatus@geelongostomy.com.au for additional information on postal charges.

Any money held in your former Association’s credit account is not able to be transferred, you will need to contact your former Association to discuss a refund. 

We ask members to adhere to the member’s code of conduct, which is available to download in the documents section.

  • Place your orders at Order Online or post your completed order form to us.

Please note: Under the Stoma Appliance Scheme you are entitled to receive the one allowance of each of the annual items, irrespective of the number of Associations you may have ordered from. You are also only entitled to one order per month from any Association.
